Cаse #1 - Yоur pаtient is new tо yоur prаctice. He is a 46 year old white male of average height and weight. He indicates on the health history that he has sinus problems due to allergies and had a knee replacement done 10 years ago in another state after chronic issues related to a football injury. He has a history of irregular dental care, and has not had his teeth cleaned "in a few years", explaining that he had a prosthetic joint infection in his knee seven (7) years ago after having a dental procedure done. He is concerned about it happening again and is"just one of the reasons" he has avoided keeping up with regular dental care. His vital signs are pulse 88, respirations 20, and blood pressure 156 / 86. He says he is noticing bleeding of his gums when he brushes, but tries "not to brush too hard so it doesn't happen". He has a three unit bridge in the UR anterior area involving #6, 7, 8 and several other amalgam and composite restorations. There is an obvious cavitated lesion adjacent to an amalgam filling on #3 occlusal.Question: While probing, his gums are bleeding heavily. He says it "really hurts" and he is visibly in a lot of discomfort. You ask if he'd like to stop. He says he 'will tough it out' but wants to sit up for a minute and take a break. Upon sitting him upright, he becomes unresponsive. What has most likely occurred?
