The pаtient is а 55 yо femаle admitted tо a skilled nursing facility after a prоlonged hospitalization for chest pain.  The patient was diagnosed with 4 vessel coronary artery disease and aortic valve insufficiency and underwent a coronary artery bypass graft x 4 and an aortic valve replacement.  Upon discharge from the hospital, the pt required mod assist for bed mobility, max assist for sit to stand transfers due to sternal precautions, and can ambulate 20’ with RW and minimal assistance. Pt becomes short of breath (SOB) with minimal exertion which is why gait distance is limited to 20’.  Pt lives with her dog, Maddie, in a 3-story home with stairs. Answer the following questions regarding completion of the ICF. Name two (2) body structures/functions that are impaired and at least one outcome tool you would use to objectively measure function. Name two (2) environmental factors that you will need to consider with this patient. Name one (1) facilitator and one (1) inhibitor that you will need to consider with this patient.
