Solu-Medrol, magnesium sulfate, and breathing treatments with albuterol did not open the airway so that he could breathe. He then became combative because of lack of oxygen. Lidocaine and ketamine were given in preparation for a rapid response intubation. Then, one of the nurses gave succinylcholine, which immediately paralyzed the respiratory muscles. It was contrary to hospital policy for the nurse to administer succinylcholine, instead of the physician who would do the intubation. No one started bagging the boy for 4 minutes, and it took almost 20 more minutes to intubate him, during which time he went into full cardiac arrest. He was successfully resuscitated but as of this afternoon appears to be suffering from some lingering brain injury related to the lack of oxygen (Dunham-Taylor & Pinczuk, 2010, p. 286).As the nurse administrator, you oversee this department along with the intensive care unit. The manager of the unit, who reports to you, is very upset, and has just informed you of this case which occurred this morning. Answer the following seven questions in the spaces provided.
