Marfan syndrоme is inherited as a dominant trait and results from a defective gene that codes for fibrillin-1, a protein critical in the extracellular assemblage and maintenance of elastin fibers. It also appears to play an important, although not well-understood, role in preventing premature breakdown and removal of elastin. Thus a person with a defect in fibrillin-1 actually has two problems. They cannot efficiently assemble elastin in the extracellular matrix and the elastin made is prematurely degraded and removed. Although found throughout the body, elastin fibers are especially abundant in the aorta and suspensory mechanism that holds the optic lens of the eye in place. A deficiency of elastin in the wall of the aorta leaves it weak, eventually leading to aneurysm formation and even rupture. Weakness of the suspensory ligaments of the ocular lens will ultimately lead to dislocation of the lens and loss of vision.
