SCENARIO 3 Yоu аre dispаtched tо а multiple vehicle accident invоlving three (3) vehicles on a 2-lane highway.  Two (2) of the vehicles have rolled off the road and into a nearby field.  You are the first medical resource on scene, as law enforcement has already stopped traffic.  You immediately see four (4) patients lying in the road, scattered from each other over a 20-foot area.  Two (2) additional patients are trapped in one of the rolled vehicles that is still overturned, and one (1) patient is in the second rolled vehicle; however, it is upright, and the doors are open.  Finally, there are an additional three (3) patients who are walking around the vehicles, not following directions from law enforcement.  (8 points total) The following is provided for your situational awareness:Green - walking wounded, alert, orientedYellow - altered, can follow basic commands, ok for delayed transportRed - unconscious or unable to follow commands, serious uncontrollable hemorrhage/no radial pulse, breathing over 30/min with/without adjunct/positioningBlack - Unresponsive to positioning, no respirations, no pulse, injuries incompatible with life 3A. Given this initial scene size-up, what sort of resources are you needing for this situation? (1) 3B. After the arrival of a supervisor, you are placed in charge of triaging the patients, and you begin with the four (4) patients in the road.  Level each of the following four patients: (1 point each) Pt. # Patient description: Leveling Color: 1 60 y/o male, breathing, unresponsive to painful stimuli, amputation to lower left leg   2 30 y/o female, responsive to painful stimuli with abnormal flexion, breathing at 30/minute and irregular, severe crepitus felt on posterior skull with an open skull fracture noted on the left parietal side with exposed brain matter   3 14 y/o female, breathing at 30/min and regular, severe deformity to both upper extremities, serious external hemorrhaging in left upper leg, no radial pulse.   4 26 y/o male, breathing is regular amid seizure activity, serious contusion observed over his frontal bone with edema over both eyes, flail chest on right side, R arm deformity, and bleeding noted from his mouth   3D. After being extricated, you are presented with the two (2) patients that were trapped, as well as the one (1) patient in the 2nd rolled vehicle.  All three were able to walk unassisted to you; however, all three appear clearly altered and cannot answer simple questions.  Each has a broken upper extremity as well as bleeding from the scalp, but is otherwise uninjured.  How would you level them and why? (1)

SCENARIO 1Yоu аnd yоur pаrtner respоnd to а rural address for an “unconscious” patient. The reporting party called EMS after she found one of the bar patrons in his vehicle after closing approximately 0230 hours. The patient was said to have left the bar around 2200 hours after several beers and cocktails. The outside temperature is 10° F. The patient is unresponsive to tapping and shouting at his window. The window is broken, and the patient is found to be breathing with shallow respirations and a slow, weak carotid pulse. (5 points total) 1A. Describe how you would move this patient out of the vehicle: (1) 1B. While in the back of the ambulance, you expose the patient to find no gross deformities or abnormalities. When assessing the hypothermic patient's pulse, how long should you check it for?  (1) 1C. Once in the back of the ambulance, you find the patient has lost his pulse. Compressions are started, and the defibrillation pads are applied. The patient is in Ventricular Tachycardia (V-Tach)  without a pulse.YES/NO: Should this patient be defibrillated? (0.5)YES/NO: Should the patient subsequently be defibrillated if he remains in VT? (0.5) In response to your treatment above, the patient obtains ROSC but remains cold, bradycardic, and hypotensive. What treatments would you provide for the duration of care? (2 points)
