A 34 yeаr оld mаle presents tо yоur clinic with left аnkle pain that started acutely over the weekend when he was playing basketball and jumped up, then fell as he landed. His left ankle and lower calf are swollen with bruising on the lateral side. Physical exam reveals tenderness to palpation of the lateral left ankle ligaments and the heel, and antalgic gait and pain with weight bearing. No tenderness is noted over the lateral malleolus. With the patient lying prone, you notice that he can actively plantarflex both feet, but cannot plantarflex against resistance with his left foot. When you place the knees at 90 degrees and squeeze the left calf, there is absence of plantar flexion, but when performed on the right leg, plantar flexion occurs; also while resting in this position, his left foot is more dorsiflexed. What is the most likely diagnosis?
