Upon further questioning, she admits to frequent bouts of depression, lack of energy and states “I have no joy left in life.” She feels these symptoms are worsening over the last 6 months, despite a supportive husband and family. Upon reflection, she has had depressive feelings “all my life really.” Her work as an office manager is stressful and she is losing weight from lack of appetite (15 pounds in 6 months). She does not smoke, has no allergies, has had a tubal ligation, and is on no medications except the occasional vitamin when she remembers to take it. Both her parents were alcoholics, but she does not drink. Her annual at your office was 3 months ago and was normal. What exam would you do today? What is your assessment and plan?
