He claims he brushes with a hard bristled manual toothbrush only if he remembers. He has never flossed and occasionally rinses with an over-the-counter mouth rinse. He has lost several teeth due to gum disease and has been told by previous dentists that he has “bad gums.” He consumes soft drinks and high energy drinks several times daily. He reports no history of smoking cigars or cigarettes. Extraoral Examination: Palpable submandibular lymph nodes were found on the right side. No other abnormalities were detected. Intraoral Examination: A small rounded vesicle was noted in the labial mucosa adjacent to tooth #12. He indicated it was painful and had been there for a few days. Gingival tissues were generally pink and stippled in the maxillary anterior regions, however there were numerous areas of gingival inflammation present in posterior and mandibular areas. Moderate to heavy calculus was visible, particularly in the mandibular anterior teeth.
