Summаry оf the cаse: A 20-yeаr-оld male fell оff a roof and suffered abdominal trauma. Paramedics at the scene made the following observations: - Abdominal pain, right upper quadrant - Cyanotic, cool, and clammy skin - Falling blood pressure, 100/60 mm of Hg - Rapid pulse, 100 beats/min Paramedics infused 0.9% NaCl (normal saline) and transported him to a rural hospital. There he was infused with O- packed red blood cells (PRBCs) and airlifted to a trauma center. At the trauma center, the following notes were added to the patient's chart: - Abdomen firm and distended - Blood type A+, A+ PRBCs administered - Positive FAST scan (focused assessment with sonography for trauma) for intraperitoneal fluid He was prepared for surgery, which revealed a lacerated liver. The laceration was repaired, and the patient's vital signs stabilized. Questions: 1) What are the life-threatening complications of rapid, substantial blood loss? How would a saline infusion help in this situation? 2) What problem does the PRBC infusion address that the saline infusion did not? 3) Why did the physician at the rural hospital use O- PRBCs before he was aware of the patient's blood type? 4) What type of blood is the patient safely able to receive? 5) What are the consequences to the patient if he receives mismatched blood? What signs and symptoms would you look for that suggest such an outcome?
