Pleаse use this pаtient cаse tо answer the fоllоwing two (2) questions Patricia is a 23 yo female presenting to UHS with complaints of N/V and diarrhea.  Her symptoms started two days ago on the plane trip home from a spring break cruise.  Her other complaints include dizziness, dry mouth, and decreased urination.  On physical exam, her tongue is furrowed and she has decreased skin turgor.  She remains nauseated and has vomited twice since presenting to the ED.  Today’s vital signs:  BP 98/58 mmHg and HR 104 bpm (previous vital signs from her physical last fall were BP 118/68 mmHg and HR 81 bpm) Today’s weight:  56 kg (previously recorded weight 59 kg) Today’s lab results:  Sodium 149 mEq/L (normal 137-144 mEq/L), serum osmolality 305 mOsm/kg (normal 278-298 mOsm/kg), and serum creatinine 1.3 (females 0.55-1.02 mg/dL)

CT is a 69-year-old white male with a 6-year history of hypertension that has traditionally been difficult to control.  He is taking the following medications (below).  At his visit today, his blood pressure is low, and he has been experiencing symptoms of hypotension (dizziness and lightheadedness).  His BP values are listed below.  The team physician wishes to discontinue one of his medications to relieve some of his low blood pressure and symptoms.  Based on the ACC/AHA guidelines, which medication is NOT a first-line therapy and should be the medication that is discontinued (following appropriate discontinuation guidance) today? Blood pressure today:  102/58, 96/54 mm Hg Heart rate:  89 bpm Medications: Amlodipine 10mg daily Lisinopril 40 mg daily Metoprolol tartrate 25mg twice daily Chlorthalidone 25 mg daily
