CASE 4Juаnitа is а 32 year-оld primagravida. During her pregnancy, the оbstetrician was оncerned about indications of inadequate uterine growth. At 39.5 weeks, she was induced and ended up giving birth via Cesarean section. Her baby was taken directly to the nursery for observation, but the staff was concerned about issues of respiratory distress and possible cardiac complications and the baby was soon transferred to the level 4 NICU at a near-by hospital about 30 miles away. Meanwhile, Juanita was unable to maintain her blood pressure post-surgery and spent one night in the ICU. You are the Lactation Care Provider at the delivery hospital and visit Juanita in her room on the third day postpartum. She is dressed in her own nightgown and robe. Her mascara runs down her face as she is crying. She tells you, “Everything has gone wrong. I wanted such a sweet and beautiful birth for my lovely baby. I imagined it for months. I looked forward to holding her skin-to-skin and having her look into my eyes first, before anyone else. Now that moment is lost forever.” She tells you that her partner is also disappointed. He is working today in the hope that he can take off extra time when the baby comes home. When you ask Juanita why she thinks she has let her partner down, she explains that he attended childbirth classes with her and was prepared to support her through a vaginal delivery and the lovely time bonding with this baby, their first after 11 years together. You learn that their plans include an all-inclusive trip in the near future. Her partner has been awarded this as a prize for his performance at his company, and he was really looking forward to showing her and the baby off on this trip. Juanita says that she is concerned because of the complications with the birth of the baby that their plans are now in jeopardy. You see that there is a breast pump in Juanita’s room and ask her if she is planning on breastfeeding her baby. She says that everyone has told her that would be best for her baby, especially because he is in the NICU. She was conflicted during her pregnancy about breastfeeding because she had heard that her breasts could lose their shape and sag. The staff had mentioned to her that you (the LCP) would come in and discuss pumping with her before she went home. Juanita says one of the staff members told her not to bother for the first few days because her milk would not come in for a few days anyway. She is scheduled to be discharged today.
