(pleаse use this cаse tо аnswer the fоllоwing four (4) questions) PZ is a 60 yo female presenting to clinic today to establish care after moving to Madison.  Aside from occasional headaches, they are complaining of SOB climbing more than one flight of stairs, and they can no longer exercise as long as they have in the past.  They are still able to complete daily household chores and hygiene activities without symptoms.  PZ requires 2 pillows at night to sleep comfortably and often feels bloated. PMH:  HTN, CCD (s/p NSTEMI and CABG x 3 vessels in 2019), migraine headaches NKDA Current meds :  Aspirin, amlodipine, atorvastatin, nitroglycerin 0.4mg SL PRN, and naproxen sodium PE:  VS:  BP 132/86 mmHg, HR 82 bpm                 Abd: (+) JVD, (+) HJR Wt: 64 kg (weight gain of 8# in 2 weeks)               Extremities:  2 (+) pitting edema in feet and ankles Heart: (+) S3       Diagnostic testing:  ECG: NSR; ECHO: EF 30%; CXR: Cardiomegaly Labs:  Sodium: 136 (136-145 mEq/L), Potassium: 4.1 (3.5-5.1 mEq/L), Magnesium: 2.1 (1.6-2.6mEq/L), BUN: 14 (7-19mg/dL), SCr: 0.8 (0.55-1.05 mg/dL), Glucose: 101 (70-99 mg/dL)
