Yоu аre wоrking in the micrоbiology lаborаtory and received an order for an Aerobic Culture, Sputum. You review the patient information and determine that the patient is a 10 year old male with cystic fibrosis (CF). You plate the sample on blood (BAP), chocolate (CHOC) and MacConkey (MAC) agar. The mucoid-appearing organism on BAP has a greenish, metallic sheen, flattened colony morphology and spreads out over the agar surface. CHOC agar plates contain a large, gray organism with a mucoid morphology, in addition to normal oropharyngeal flora. The MAC agar plates contain a clear, mucoid, non-lactose fermenter (image below). Colony gram stain of the BAP, MAC, and CHOC colonies all show a Gram-negative rod. Triple sugar iron agar testing reveals a glucose and lactose non-fermenter (TSI K/K; alkaline slant and butt) that is negative for gas and hydrogen sulfide production.
