Yоu аre the nurse wоrking in the emergency rоom, it is the beginning of your shift 0712 аnd EMS аrrives with a 63 year old female patient with right sided arm weakness, rt facial droop and difficulty speaking.  Per EMS, the patient’s husband is parking the car and will be in shortly.   History and Physical   The patient is a 63 year old, African-American woman with a history of Diabetes Mellitus type II, hypertension and rheumatoid arthritis.   She works as an attorney in a busy office and lives at home with her husband.  Due to aphasia, her husband is answering questions for you at the bedside.  According to the patient’s husband, the patient has been having some increased stress recently related to work and to his knowledge has not been monitoring her blood pressure.  He is able to describe the happenings of the morning and the night prior.  He states that the patient was in her usual health the night prior and went to bed around 2200.  As per their routine, they woke around 0500, the patient walked her dog and came back inside.  As she was walking past the coffee pot, she poured 2 cups of coffee and carried one in each hand to the table and sat with her husband to watch the 6:00 news.  She was able to converse normally with her husband and he did not notice any slurred speech or facial drooping.  At approximately 0630 she went to her vanity as she normally does to apply makeup and get ready for work when she called out to her husband.  While attempting to apply makeup she noticed that she wasn’t able to hold her makeup brush with her right hand.  Her husband came to her side and notice that her face was drooping.  Noticing that this was a sign of stroke, her husband call 911 and she was transported to the Emergency Department.   Nurses Note  07/05/2025  0746 – Patient has returned to the emergency department from radiology after stat CT and CTA.  Prior to the CT her accuchek was 124.  She continues to exhibit unilateral weakness on the right side, right side facial droop and slurred speech.  She is tearful and husband remains at her side.  The emergency room provider arrives to the room along with the neurologist and discussed thrombolytic treatment with the patient and her husband.  The patient and husband are agreeable to the thrombolytic treatment and the neurologist has the consent signed.  The patient is placed on the telemetry monitor and her rhythm shows atrial fibrillation with a rate of 102.  Vitals are as follows: Weight 176 lbs. BP 184/122 P 102 R 24  O2 sat 97%.  NIHSS = 10    After the physician leaves the room, the patient is able to speak a few words and tells you that she has a history of atrial fibrillation and also had her gallbladder removed 4 months ago. The husband, who is emotional and stressed, then remembers this and apologizes for not telling you earlier.  The patient begins to moan and hold her head and complains of extreme pain in the back of her head.
