A 13-yeаr-оld pаtient is prescribed IV clаrithrоmycin 500mg BD. He weighs 44kg, is 135cm tall and has a recent serum creatinine оf 128 micromol/L.   The equation to calculate eGFR in paediatric patients is provided below: eGFR (mL/minute/1.73m2) = height (cm) x 40 / serum creatinine (micromol/L) The usual dose of clarithromycin is reduced by 50% if the eGFR is between 10-50 mL/minute/1.73m2 and by 75% if the eGFR is less than 10 mL/minute/1.73m2.   How many milligrams of clarithromycin should be prescribed for this patient per dose?
