Cal's difficulties first began with trouble falling asleep and frequent nightmares. He is preoccupied with morbid daydreams of family members' dying or being injured and of threatening and terrifying antisuperheroes.He refuses to go to bed unless his mother sits beside him, and most nights he will fight sleep for more than an hour so that she will not leave him. Especially on school days, Cal complains of many headaches and stomachaches for which no physical basis could be found on a recent pediatric examination. Because of these physical complaints, he frequently stays home from school and has trouble paying attention when he does go to school. He often insists that he is sick and needs to go home. His school performance has deteriorated, and his grades have dropped from an A average to barely passing.Cal's social relations have also become increasingly restricted; he is reluctant to play away from home at friends' houses and absolutely refuses to sleep at a friend's house overnight. Nonetheless he is glad to have friends come to play at his house. When he does go to a friend's house, he insists on writing down the phone number at which his mother can be reached and insists that his mother tell him the exact time she will be picking him up. He frequently asks to call his mother to reassure himself that she is there. He also repeatedly asks what time it is and how long it will be until his mother is supposed to come. If she is by some chance delayed by even a few minutes in picking him up, he becomes fretful and unable to focus his attention on activities with his friends.The interview shows Cal to be a worried, anxious, sad, child with difficulty attending to the examiner. He brightens up immediately, however, whenever his mother returns to the room. Cal describes his multiple problems with difficulty and says that he is afraid matters will never improve, although he does relate well to the examiner and states a desire to feel better and to attain his previous level of school achievement. There is no evidence of a thought disorder, cognitive impairment, or suicidal risk.
