The husband says the patient woke this morning complaining of some chest discomfort and having difficulty “catching her breath.” Upon standing, the patient had a syncopal episode and that is when her husband called the paramedics. Her husband states that the patient has no past medical history and the only medication she is taking is oral contraceptives. There is no previous family history of sudden cardiac death. The patient is an active smoker. Cardiopulmonary resuscitation (CPR) is continued upon patient arrival to the ED. She is placed on the cardiac monitor and during a rhythm check the patient is found to have a bradycardic rhythm, however, she remains pulseless. CPR is subsequently resumed and additional 1 mg of epinephrine is administered. Return of spontaneous circulation is achieved. An ECG is performed which reveals sinus tachycardia with a right bundle branch block and right axis deviation but is otherwise unremarkable without evidence of ST changes. What is the likely cause of the patient’s presentation?
