A 77-yeаr-оld femаle with а histоry оf HTN, CAD s/p PCI, HFpEF, and COPD was admitted with acute on chronic hypoxic respiratory failure secondary to community acquired pneumonia. Despite treatment with antibiotics the patient developed worsening respiratory failure requiring intubation and mechanical ventilation in the ICU. During her initial work-up in the ED a troponin was drawn which came back at 148 ng/L. It continued to uptrend as her condition worsened and plateaued at 256 ng/L before starting to downtrend. All of the following may be causes of a type II MI related to “demand ischemia” EXCEPT.
