A 68-yeаr-оld mаn presents tо the clinic with prоgressive shortness of breаth over the past month. He reports difficulty breathing when lying flat at night and has been using two pillows to sleep comfortably. He also notes swelling in his ankles and feet, which has worsened over the last few weeks. The patient describes fatigue and decreased exercise tolerance, stating that he becomes winded after walking just a few blocks. He denies chest pain or palpitations. His past medical history is significant for hypertension and a myocardial infarction five years ago. He takes lisinopril, atorvastatin and metoprolol. Social History: denies drugs, alcohol and quit smoking 5 years ago  On physical examination: Blood pressure: 142/86 mmHg Heart rate: 94 bpm Respiratory rate: 20 breaths/min Oxygen saturation: 94% on room air Jugular venous distension is noted Bibasilar crackles are heard on lung auscultation 2+ pitting edema is present in both lower extremities Heart sounds reveal a displaced apical impulse and S3 gallop Based on the history and clinical presentation, what is the most likely diagnosis?
