Nikki is a 70 yo male admitted to the acute care hospital with Covid 19.  His PMH was significant for COPD, DM, obesity, and HTN. On day 5 of his admission, he was intubated and required mechanical ventilation due to developing acute respiratory failure. Over the last 2 weeks, Nikki made significant process. He was extubated and is now admitted to your post-acute center for rehab. His vitals are: Supine: HR: 90 bpm; BP:148/88; RR:18; O2 Sat: 96%; RPE 1 You sit him up and is vitals in sitting: HR:98 bpm; BP 134/80; RR  20; O2 Sat: 95% RPE 2 You decide to stand him after sitting for 3 minutes. His vitals in sitting: HR: 104 bpm; BP 118/80; RR 24; O2 Sat 95%; RPE 3  You have him stand for 3 min and reassess his vitals: HR:102 bpm; BP 140/82; RR 20; O2 Sat 95%; RPE 3 Nikki’s vital sign response is most likely consistent with the following conditions:
