She was discharged at 24 hours from the hospital and no jaundice was noted. Include pertinent positive and negative data from the case. Weight today is 7 pounds 2 ounce. She initiates breastfeeding every 2–3 hours and has a pale wet diaper and yellow stool after almost every feeding. Her maximum alert period is 1 hour and she rouses easily. Physical exam is normal except for jaundice to the level of the nipples. Total serum bilibrubin today is 13.2. The bilirubin nomogram is included for reference. Phototherapy Thresholds AAP Given the information in the case and using the bilirubin nomogram: Discuss additional information you would want to obtain regarding prenatal,  birth, or neonatal history and provide rationale. (1) Identify the most likely diagnosis (be specific), note infant’s risk level based on information from the case, and provide rationale for your answers. Note pertinent positive and negative data from the case. Use nomogram and knowledge base to support your answer. (2) Describe the best plan of action for this child right now and specify recommended follow up and parent education; include rationale for your actions (2)
