The nurse cares for a 76-year-old male admitted to the Emergency Department with reports of lightheadedness, fatigue, and chest palpitations. Nurses’ Notes 1000. 76-year-old male admitted to the ED with lightheadedness, fatigue, and chest palpitations. History of Type 2 diabetes mellitus (treated with metformin), recent lower GI bleed, hyperlipidemia, and hypertension. Non-productive, persistent dry cough present. Lung sounds with rales in the bases bilaterally. Shortness of breath with exertion. Skin pale, warm, moist, and dry. Oriented x2, reports nervousness. Last BM this morning with no evidence of frank blood. Voiding with no difficulty. Poor appetite over the last 2 days. 2+ edema to the lower extremities. No nausea or vomiting. Stupidly did not take daily meds this morning. EKG: Chest XRAY: Bilateral pleaural effusions and cardiomegaly Laboratory Report Lab Results Reference range Cholesterol (Total) 250 mg/dl
