An 81-yeаr-оld femаle with а histоry оf dementia is brought in from a nursing facility for “not acting right” and decreased responsiveness. Staff report she has been more lethargic and eating less over the past two days but cannot provide further history. On assessment, she is disoriented, moaning intermittently, and unable to localize pain. Vital signs show HR 118, BP 92/58, RR 24, and a temperature of 99.5°F. Her skin is cool with mottling noted over her lower extremities, along with scattered petechiae on her arms and abdomen. Capillary refill is delayed. Distal pulses are present but weak bilaterally.
