Pathophysiology: ↓ Renal blood flow → ↓ GFR → ↑ BUN/Cr ratio. Activated RAAS and sympathetic nervous system → sodium/water retention, concentrated urine. Common causes: Volume depletion: hemorrhage, GI losses, diuretics, third spacing. Decreased effective perfusion: heart failure, cirrhosis, sepsis (early). Renal vasoconstriction: NSAIDs (↓ prostaglandin dilation of afferent arteriole), ACE-I/ARB in renal hypoperfusion (↓ efferent arteriolar constriction). 2. History Symptoms of hypovolemia: dizziness, thirst, decreased urine output. Fluid loss history: vomiting, diarrhea, poor intake, diuresis, burns, trauma. Cardiovascular clues: heart failure symptoms (dyspnea, edema), hypotension. Medication clues: NSAIDs, ACE-I/ARB, diuretics. Risk factors: older age, CKD, volume depletion states. 3. Exam Findings Volume depletion signs: Tachycardia, hypotension, dry mucous membranes, decreased skin turgor. Orthostatic hypotension. Volume overload (low effective perfusion): peripheral edema, JVD (heart failure), ascites (cirrhosis). Oliguria: often 20:1 (classic). Urine Na⁺
