The patient assigns a pain rating of 4 out of 10 which is worse with walking.  Four days ago the patient returned to Florida following a trip Hawaii.  The patient has no known drug allergies Current medications: low dose oral contraceptives for treatment of perimenopausal symptoms Vitals: B/P 114/70; Pulse 64 regular; Respirations 16 and unlabored; Temperature 99.2; BMI 34.5 kg/m2 Physical exam: Right calf is slightly larger than the left and warm to touch. Increased pain is elicited on dorsiflexion. Obvious varicosities are noted bilaterally. 2+  edema is observed on the right leg from the calf down. The remainder of the physical exam in unremarkable. Laboratory diagnostics: Highly sensitive d-dimer test is positive.  What is the appropriate clinical management plan for this patient at this time?
