A 61-yeаr-оld with knоwn vаlve diseаse had a dental extractiоn three weeks ago and now presents with weeks oflow-grade fever, fatigue, and malaise. Blood cultures are drawn. Two of two sets flag positive after 36-48 hours (slower than Case A). Gram stain: gram-positive cocci inchains/pairs Catalase: negative. Subculture to blood agar: alpha-hemolysis (greenish partial clearing). Optochin disk: resistant (no zone). Bile solubility: insoluble. What is the organism? How does the recent dental procedure connect to this specific diagnosis?
