The nurse in the emergency depаrtment (ED) is cаring fоr а 56-year оld female.  Review the nurse's nоtes and select which findings require immediate follow-up by the nurse. Select all that apply. Nurse's notes 1400 - Client presents to the ED with dyspnea and dizziness and feeling as if she may “pass out”. She reports her morning bowel movement appeared to be black and resembled tar.  The client reports an exacerbation of arthritis for which she has been taking ibuprofen every 6 hours but is otherwise healthy. Significant other at the bedside reports the client has appeared less active and fatigued for several weeks. Vital signs: BP 90/54, HR 105, RR 30, T 98.9° F (37.2°C), and pulse oximetry reading of 89% on room air. Upon assessment, the nurse notes lung sounds are clear throughout all lung fields. Peripheral pulses are 2+. S1, S2 present, no extra heart sounds heard. Skin is cool to touch and pale. Capillary refill is less than 3 seconds. The client is alert and oriented x 3.  Reports “burning” upper abdominal pain rating 4/10. Home medications include ibuprofen 600 mg every 6 hours as needed for joint pain.  Nonsmoker. Occasional social drinking of no more than 2 alcoholic drinks at a time.
