Scene Upon arrival, the patient is found seated in a recliner, spontaneously awake, but fatigued; able to speak in short sentences. Skin is pale and cool. During assessment, the patient becomes increasingly lethargic, gag reflex still intact. Vital signs: BP 92/60, HR 101, RR 24, SpO₂ 95% on room air, BGL 48 mg/dL. Post Scene 12-lead shows signs of ischemia. The patient becomes more alert after glucose administration, but now reports a sensation of constant "chest pressure."
