Questions

This exаm cоntаins 5 questiоns. Yоu must choose 2 questions to аnswer. All questions are equally weighted. Below, you will see a list of all 5 questions. After that, there are two answer boxes—one for each of your chosen questions.Use one box per answer. Since there are only two boxes, you only need to answer 2 out of the 5 questions. Within each answer box, please ensure you tell us which question you are answering. For example: 'I am answering Question 1'. Question 1: The following test results were obtained from a 47-year-old man during a routine health check. Recently, the patient has reported feeling increasingly tired and has experienced some weight gain, which he attributes to a heavier workload. Using the patient’s adrenal function test results, explain the biochemical mechanisms responsible for the level of these adrenal hormones and propose a potential diagnosis (40%). Discuss an appropriate testing approach and any additional tests that could aid in confirming your diagnosis (40%) and outline the clinical manifestations that may be expected in a patient with this diagnosis and describe any potential treatment options for managing this condition (20%).   Question 2: A 42-year-old woman presents to a GP clinic with a visibly enlarged and palpable thyroid gland on examination. Using the thyroid function test results provided, identify the type of thyroid disorder affecting this patient (10%) and explain the biochemical mechanisms responsible for the observed thyroid hormone levels (30%). Describe any additional tests that could be performed to confirm the diagnosis and differentiate this condition from other thyroid disorders (40%). Finally, discuss the clinical symptoms that may be associated with this disease and outline possible treatment options for its management (20%). Question 3: Use the following clinical and biomarker data provided for two patients with melanoma to assess their risk of tumour progression (20%). Discuss the relevance of these data and any other biomarkers of melanoma you might be aware of for the screening, diagnosis, prognosis, stratification and monitoring of melanoma (50%). Propose a potential management plan for each patient based on the data provided (30%). Question 4: Phenytoin is an anti-convulsant drug used in the treatment of epilepsy and is commonly monitored in clinical biochemistry laboratories because of its complex pharmacokinetics. Explain when therapeutic drug monitoring (TDM) may be required in the clinical management of patients receiving phenytoin (10%). Discuss the key pharmacokinetic parameters that influence circulating drug concentration and clinical response, explaining why the elimination of phenytoin follows Michaelis–Menten kinetics and the clinical significance of this for dose adjustment and toxicity risk (60%). You should also explain the pharmacodynamics of phenytoin, including its mechanism of action and how this relates to its therapeutic and adverse effects (30%). Question 5: A 30-year-old woman presents to the emergency department 10 hours after ingesting 20 grams of paracetamol in an apparent suicide attempt. She complains of nausea and abdominal pain. Interpret the biochemical results and clinical observations above in the context of paracetamol toxicity (40%) and discuss a potential management plan for this patient (10%). Explain the clinical pathophysiology, biochemical features of poisoning and methods of analysis for one of the major drugs of abuse (50%).

When distinguishing between jоb sаtisfаctiоn аnd оrganizational culture, we can say that job satisfaction is descriptive, whereas organizational culture is evaluative.
