A 45-yeаr-оld mаle with а histоry оf polycythemia vera presents with recurrent abdominal pain, splenomegaly, and recent onset of gastrointestinal bleeding. Lab results show platelets at 54,000/µL (thrombocytopenia), suggesting hypersplenism. Liver function tests are nearly normal, and the patient has no history of liver cirrhosis.  Imaging is performed: Doppler Ultrasound: Shows absence of the main portal vein, which is replaced by multiple, small, serpiginous venous structures (worm-like) in the porta hepatis. These vessels demonstrate hepatopetal (towards the liver) flow. CT Scan: Confirms chronic thrombosis of the main portal vein with periportal collateral vessel formation. What is the most likely diagnosis?
