Cаse study pаrt 1 Betty, аge 67, had a tоtal left-hip replacement secоndary tо osteoarthritis. She lives with her husband on a First Nations reserve in a small northern Canadian community. Betty had her hip replacement surgery in the orthopedic unit of a large acute-care hospital in the downtown area of a major city. She was discharged to her home on the First Nations reserve three days post-surgery. At four days post-surgery, Betty is experiencing mobility issues and pain related to the osteoarthritis in her right (other) hip. You have been practicing as an occupational therapist in this northern region for two years. You receive a referral to assess Betty at her home. The client record indicates: · Betty was transferred home one week ago and is walking slowly around the house with a two-wheeled walker. · While in the hospital, the health care team discovered that Betty has type 2 diabetes. A nurse will be visiting Betty at home once a week to monitor her blood glucose levels. Betty’s diabetes is currently being managed with oral medication. · Betty is maintaining hip precautions and weight-bearing status on her leg. · Betty knows how to use a cane to get up and down stairs. · Betty needs help with sponge bathing; she is not allowed to shower until the staples are removed. Betty uses adaptive aids for lower body dressing. · Betty’s daughter is visiting for the three weeks to help her. · Betty’s husband says he is going to help with the chores around the house while she heals. · Before the surgery, Betty was active in the community, visited friends frequently and attended community gatherings. · Betty answered the phone when you called to schedule the appointment. Betty gave you directions to her home. Self-Study Exercise for Part 2 Instructions: Read the additional case information below and, 1. Identify the relevant assessment data that is presented in the case study. 2. Identify the client's occupational performance issues. 3. Determine the services you would recommend for this client. You are provided 30 minutes to complete this self-study exercise. Following this, the assessor will take you through a series of oral questions related to the case. Case study part 2 You have completed an initial occupational therapy assessment with Betty, and you have determined the following: General information: Prior to her surgery, Betty loved to go out for coffee with her friends and attend social gatherings. However, since her surgery Betty is reluctant to leave the house. Betty’s daughter is staying with her for 2 weeks to help her. When her daughter leaves in two weeks, no one will be available to make Betty’s meals. Betty would like to make some light meals for herself as soon as possible because she doesn't want to “burden anyone.” Betty's husband works part-time. There is no community support for meal or grocery delivery. Betty’s freezer is not working properly. Betty prefers to shower rather than having a sponge bath. Betty has difficulty walking and moving around outside on the back deck, and in the laundry room and garage. Betty has been sleeping in her electric recliner/lift chair. Betty is an active member of the First Nations community. Physical assessment: Betty stated that she has had some pain and swelling around her surgical incision and in her left leg. Betty can only walk for 15 meters. Her ability to walk farther is limited by her right (other) hip pain. She reported pain in her right hip as well as swelling in her left calf and lower leg. Home assessment: Betty and her husband live in a split-level home (a home with multiple levels connected by short flights of stairs). There are three stairs outside leading up to the entrance of the home. There is a hand railing along the stairs, but it is unstable. Once inside the house, there are five steps from the entrance to the main living area. The stairs do have a handrail. Betty’s bedroom and a bathroom are also on this level. The laundry facilities are in the basement, which is down five stairs. The bathtub has one grab bar. Betty has borrowed a transfer bench for the bathtub, but it does not fit properly. The toilet seat is raised with arms. Betty’s family has removed the bathroom door. Betty’s bed is too low. The occupational therapist in the hospital ordered blocks to raise the bed, but they have not arrived yet. There is carpeting in the main part of the house and in the bedroom. There is linoleum flooring (a hard, smooth and washable floor surface) in the kitchen.
