Nurse-led mindfulness interventions have shown promise in adult populations, but evidence in adolescents remains limited. Methods: A randomized controlled trial was conducted with 84 adolescents (ages 13–18) recruited from two pediatric endocrinology clinics. Participants were randomized to either a 6-week nurse-facilitated mindfulness program (n = 42) or standard care (n = 42). Anxiety was measured using the Generalized Anxiety Disorder-7 (GAD-7) scale at baseline, immediately post-intervention, and at 3-month follow-up. Secondary outcomes included HbA1c levels and self-reported diabetes self-management behaviors. Data were analyzed using repeated-measures ANOVA. Results: Intervention participants demonstrated a significant reduction in mean GAD-7 scores compared to controls (p < .01), with effects sustained at 3 months. HbA1c levels improved modestly but did not reach statistical significance. Self-management behaviors, including adherence to blood glucose monitoring, improved in the intervention group (p < .05). Conclusion: Nurse-led mindfulness interventions may reduce anxiety and enhance self-management behaviors in adolescents with type 1 diabetes. Larger, multi-site trials are warranted to confirm these findings and explore long-term effects on glycemic outcomes.
