Cаse Study pаrt 1 Fаtima, age 42, is self-emplоyed as a cleaning lady. Recently, she suffered a left-sided cerebral strоke. Fatima was transferred frоm the intensive care unit (ICU) to the medical unit. The plan of care involves stabilizing Fatima medically and then discharging her home with outpatient supports.   Fatima lives in a basement apartment with her boyfriend, who is unemployed. Fatima came to Canada from Portugal on a work visa one year ago. She does not have any family members in Canada. As the occupational therapist, you are asked to implement a treatment plan to support her discharge home and referral to community supports. Information in the client record: Fatima is right-hand dominant, but she has increased flexor tone in her right hand and wrist and presents with a fine motor co-ordination deficit;  Fatima is orientated to person, place and time and she does not have any cognitive deficits; Problems with balance are noted when walking and sitting; Fatima walks independently with a walker for short distances. She complains of vertigo (dizziness) and difficulty moving from a sitting to a standing position; Fatima smokes one pack of cigarettes a day and drinks socially; Fatima and her boyfriend live in a small one-bedroom basement apartment. To access the apartment, there are 12 steps without a handrail outside; The bathroom has a walk-in shower; The nursing staff have referred Fatima to social work. Self-Study Exercise Part 2 Read the additional case information below and: 1.      Determine the client’s occupational performance issues 2.      Determine your goals, as well as your intervention or treatment plan for the client 3.      Identify the client's diversity, values and/or beliefs  You are provided 40 minutes to complete this self-study exercise. Following this, the assessor will take you through a series of oral questions related to the case. Case Study part 2 As an occupational therapist you conducted your initial interview and determined: Fatima speaks English at a beginner level and is unable to read English;  The bathroom in her hospital room has a raised toilet seat; Fatima states that she tires easily; The medical record indicates that Fatima’s blood pressure remains elevated despite medications; Fatima stated that she does not have laundry facilities in her apartment; Fatima stated that her apartment has a small kitchen with a microwave and hotplate; Fatima values her independence and is frustrated that she must now ask for help. She cried through the assessment interview and does not know how she is going to manage at her cleaning job; Fatima said she "is concerned that she will have another stroke"; Fatima stated that it is very important for her to get back to work soon; Fatima is active in the Portuguese community and volunteers at the local community centre; Prior to her stroke, Fatima used public transit to go to work, the grocery store and the laundromat; Fatima has been referred for outpatient physiotherapy.
