Based on what you have learned so far in this course, organize and write the information so that it is clear, concise, and suitable for entry as an interim note. Prior to initiating interventions, the following data was gathered: The blood pressure reading was 130/85 mmHg, heart rate was 88 beats per minute, and respiratory rate was 20 breaths per minute. There was a scar that appeared to be healing on the left hip, surgical staples were present and intact, and there was no drainage from the wound. The following data were gathered during or after the interventions: The patient's left knee musculature measured at a 4+ out of 5 and the ankle musculature measured 5 out of 5. The assistant helped the patient walk in the hallway. The patient was able to walk 50 feet while the assistant provided about 20% assistance. The patient used a standard walker. The assistant provided verbal instructions for sequencing and encouragement to put as much weight as the patient felt comfortable with through the leg. The patient also occasionally placed the walker too far in front of her and the assistant had to remind the patient of the proper walker placement. When the assistant helped the patient up from bed, the patient needed 50% assistance to scoot over to the side of the bed. The assistant primarily helped by moving the operative limb. When sitting up in bed, the patient required more assistance and was able to perform about 25% of the transfers from supine to side lying to sitting. When moving from sitting on the edge of the bed to standing with the walker, the patient performed about 50% of the task.
