Explain when therapeutic drug monitoring (TDM) may be required in the clinical management of patients receiving phenytoin (10%). Discuss the key pharmacokinetic parameters that influence circulating drug concentration and clinical response, explaining why the elimination of phenytoin follows Michaelis–Menten kinetics and the clinical significance of this for dose adjustment and toxicity risk (60%). You should also explain the pharmacodynamics of phenytoin, including its mechanism of action and how this relates to its therapeutic and adverse effects (30%). Question 5: A 30-year-old woman presents to the emergency department 10 hours after ingesting 20 grams of paracetamol in an apparent suicide attempt. She complains of nausea and abdominal pain. Interpret the biochemical results and clinical observations above in the context of paracetamol toxicity (40%) and discuss a potential management plan for this patient (10%). Explain the clinical pathophysiology, biochemical features of poisoning and methods of analysis for one of the major drugs of abuse (50%).
